Composite Restorations versus Amalgam Restorations

Resin composite restorations, commonly known as tooth-coloured fillings, offer several advantages over traditional amalgam fillings, These “tooth-coloured” fillings have revolutionized the field of dentistry since their introduction in the 1960s.  This marked a significant departure from the traditional use of dental amalgam for restorative purposes, making resin restorations the preferred choice for many patients and dentists alike.

It was Dr. Raphael Bowen who introduced the first dental composite resin material in the early 1960’s.  This early composite consisted of a mixture of dimethacrylate monomers and inorganic filler particles. Initially, these materials were primarily used for aesthetic enhancements such as veneers and tooth-coloured restorations in non-load-bearing areas. These fillings were primarily used on front teeth, and typically avoided on back teeth, particularly those with heavy bites.

Over time, advancements in material science and adhesive dentistry have led to the development of stronger and more durable composite resin materials. These improvements have expanded the applications of resin restorations to include load-bearing posterior restorations, such as fillings for cavities. Composite resins have gained widespread acceptance as a viable alternative to dental amalgam for restorative procedures.  This brings us to the current day, where most practices including Dr. P Singh Inc., are now mercury-free.  We are considered “Aesthetic Dental Practices”, primarily utilizing composite resin material for most restorative treatments.  As such we do not place any amalgam restorative materials.

Let me shed some light on the reasons why we almost exclusively utilize composite resin restorative materials.

Aesthetic Appeal: One of the most significant benefits of resin restorations is their ability to match the natural colour of the tooth. Unlike amalgam fillings, which are silver or grey in colour, resin fillings can be customized to blend seamlessly with the surrounding teeth, providing a more aesthetically pleasing result. Conservative Tooth Preparation: Resin fillings allow for more conservative tooth preparation compared to amalgam fillings. This helps preserve more of the natural tooth, resulting in a stronger and more structurally sound restoration.

Versatility: Resin composite materials offer greater versatility in terms of shaping and molding the filling to match the natural contours of the tooth. This allows for more precise restoration of the tooth’s anatomy and function, leading to improved longevity and durability of the restoration.

Reduced Sensitivity to Temperature Changes: Resin fillings exhibit less sensitivity to temperature changes compared to metal amalgam fillings. This means patients are less likely to experience discomfort when consuming hot or cold foods and beverages, enhancing their overall comfort and satisfaction with the restoration.

Mercury-Free: Unlike amalgam fillings, which contain mercury, resin composite fillings are mercury-free. This is particularly important for patients who are concerned about potential health risks associated with mercury exposure. Resin fillings provide a safe and biocompatible alternative that gives patients peace of mind.

Minimal Risk of Expansion and Contraction: Amalgam fillings tend to expand and contract in response to temperature changes, which can lead to cracks and fractures in the tooth over time. Resin composite fillings, on the other hand, are not affected by temperature changes to the same extent, reducing the risk of damage to the tooth structure and improving the long-term stability of the restoration.

Enhanced Bonding Strength: Resin composite materials bond chemically to the tooth structure, creating a strong and durable bond that helps reinforce the tooth and prevent further decay. This superior bonding strength provides added protection against recurrent decay and ensures the longevity of the restoration.
